MCP server that lets agents self-report genuine failures: missing context, broken tools, capability gaps, loops, and bad tool selection. Reports persist in SQLite and are inspectable via a FastAPI web UI.
Requires Python 3.11+. After cloning, install deps with 'uv sync --group dev' (or pip), then run 'uv run agent-diagnostics run' to start the web UI and MCP endpoint (default 127.0.0.1:8765, MCP at /mcp/). Described by its author as built for educational purposes.
